Follow these steps to work towards better pelvic health, starting now:
Determine your appointment type:
PT Eval - In Clinic: 55-minute initial physical therapy evaluation for new patients of Lady Bird PT. After your appointment, a superbill will be available in your patient portal to submit for insurance reimbursement of paid sessions.
Virtual PT Eval (TX patients only): 55-minute virtual initial physical therapy evaluation for new patients of Lady Bird PT. This service is only available for patients that are in the state of Texas. If you are out of state, please use the Virtual Wellness Consult service instead. After your appointment, a superbill will be available in your patient portal to submit for insurance reimbursement of paid sessions.
Virtual Wellness Consult (out of state): 55-minute virtual wellness consultation with Lady Bird PT. This service is for any client not located in Texas. If you are in Texas, please use the Virtual PT Eval (TX patients only) service instead. This appointment is for a wellness consultation and is not eligible for insurance reimbursement. No superbill will be provided.
PT Eval - Medicare, Medicaid, Tricare: 55-minute initial evaluation physical therapy appointment for new patients of Lady Bird PT with any government-funded insurance plan. This service does not qualify for a superbill and, by law, cannot be submitted for insurance reimbursement as Medicare, Medicaid, and Tricare do not reimburse for out-of-network providers. By booking this appointment, you agree that you will not submit a claim for reimbursement for services to your insurance provider.
Select your appointment time and provider
You will be prompted to submit your information to create your patient portal.
Add your credit card information to reserve your time slot.
Create your portal password by following the link in your confirmation email titled “Registration at Lady Bird PT.”
Complete the required consent forms and new patient questionnaire ahead of your appointment, located in the Documents tab of your patient portal.
Online scheduling is not available for in-home visits. Please call the clinic to ensure you are in our service area and book your in-home visit.
Please note that we are out of network with all insurance plans and all services must be paid in full at the time of your appointment. Please check with your insurance to determine if you have out-of-network benefits. Certain services are eligible to submit for insurance reimbursement; see above for more details.

Log into your patient portal. (Still need to set up your portal? Click here!)
Select “Book an appointment” on your Schedule tab.
Choose your service and select your provider to view their upcoming availability and book your appointment. We recommend staying with the same provider for optimal care.

Lady Bird PT is out of network with all insurance providers. This is an intentional choice we’ve made so that we can provide outstanding, customized patient care dictated by your body and your goals — not by arbitrary insurance rules.
Your investment goes directly toward dedicated, one-on-one time with highly specialized doctors of physical therapy. Our average patient resolves their primary concerns in 6-10 sessions – significantly fewer than the 20+ often required in insurance settings. And there's never a surprise bill - we are transparent with our pricing from the get-go and offer flexible options to make care work for you.
But more importantly? We save you time, frustration, and prolonged discomfort.
By booking your appointment, you agree to pay the full cost of the session at the time of your appointment. Depending on the service, itemized superbills will be available in your patient portal after payment is rendered to submit to insurance for reimbursement. Please check with your insurance to determine if you have out of network benefits.

The state of Texas allows PTs to evaluate and treat patients for up to ten business days without a referral, but we will need a referral on file to continue treatment at any appointments that fall outside of that ten day window. Out-of-state wellness consults do not require a referral.
If you haven't already done so, please reach out to your provider and ask that they fax us a referral to 737-207-9386. If you already have a referral, you can upload it to your patient portal in PtEverywhere under the Documents tab or email it to us at contact@ladybirdpt.com.
We can accept referrals from an MD, DO, NP, PA, CNM (certified nurse midwife), or DC (chiropractor).
We cannot accept referrals from other PTs, CPMs (certified professional midwives), nutritionists, personal trainers, doulas, etc.
